Who is eligible?
To create an account on the CMC Patient Portal, you must be a patient of Catholic Medical Center. Currently, only patients age 18 and older are eligible to access the patient portal. You will not be able to access your child’s records using the portal.
I am not able to create an account?
If you are NOT using Internet Explorer 8 or 9 you will need to add the “cmc-nh.org” domain to the compatibility view.

Here are the steps to enable compatibility mode for the cmc-nh.org domain:

How do I activate my account?
Once the account has been authorized, we will send you an email with the instructions on how to sign into your patient portal account. Some email clients block emails from unfamiliar senders. I registered for the portal but my account cannot be activated?
We try to address these issues as quickly as possible, but the authorization process can take as long as 1-2 business days in some cases. If you would like to check the status of your request, you can contact cmcpatientportal@cmc-nh.org
It has been more than two business days and I am still not able to activate my account?

What happens if I forgot my password?
To reset your password, click the “forget password” link on the CMC Patient Portal home page. You will be prompted to enter your account username and email address. What information is available?
The Catholic Medical Center Patient Portal offers patients electronic access to a summary of their hospital visit. The summary contains medications, immunizations, general discharge instructions and results. If additional information is needed please print an Authorization Release Request Form and fax the completed authorization form to 603-663-6531. You may also visit the Health Information team on Level C of the hospital. Complete an Authorization to Release  form and hand it in or leave in the available locked box. Office hours are Monday through Friday 8:00Am - 4:30 PM.
